A woman was killed and four people were injured on August 22, 2016, when a tractor-trailer carrying a load of the propellant devices that inflate auto airbags crashed and exploded near the Quemado, Texas town on the Mexican border.

An oilfield worker was killed on an oil rig near Midwest, Wyoming on July 14, 2016. According to the initial investigation, Dennis McCulloch fell 76 feet from a workover rig and died instantly.

An oilfield worker was killed on an oil rig near Gillette, Wyoming on July 7, 2016. According to investigators, the derrick on an Anschutz Company well site moved unexpectedly, and struck and killed 25 year-old Joshua Adams. Mr. Adams was working for Basic Energy Services, a Texas company. The circumstances of the tragic death is still being investigated.

Three workers were burned and one was killed when a well site operated by XTO Energy exploded, engulfing the rig and the workers in a devastating fire. The explosion occurred on June 18, 2016 at the XTO well site outside of Watford City, North Dakota.
